    DETROIT – General Motors is recalling nearly 474,000 Chevrolet, Pontiac and Saturn cars to fix a transmission problem that can cause the cars to roll away unexpectedly.

    H&M Recalls Children's Water Bottle Due to Choking Hazard, Sold Exclusively at H&M Stores

     

    WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

     

    Name of Product: Children's Water Bottle

     

    Units: About 2,900

     

    Importer: H&M Hennes & Mauritz, New York, N.Y.

     

    Hazard: The water bottle's spout can break off, posing a choking hazard to children.

     

    Incidents/Injuries: H&M has received one report of an incident in England of the water bottle spout breaking off in a child's mouth as the child was drinking from the bottle. No injuries have been reported.

     

    Description: The 16 oz. water bottles are pink plastic with a crackle design or blue plastic. The bottles have flip-top lids in coordinated pink and blue colors. "H&M Sweden" and "www.hm.com" are embossed on the bottom of the bottle. The recalled products were manufactured in June 2012. The water bottle contains the manufacture date in an embossed date clock on the bottom of the bottle. The inner circle on the clock contains the number 12 with an arrow between the two numbers. The arrow points at the number 6 in an outer circle of numbers.

     

    Sold exclusively at: H&M stores with children's departments nationwide from July 2012 through August 2012 for about $5.

     

    Manufactured in: Italy

     

    Remedy: Consumers should immediately stop using the water bottle and return it to H&M for a full refund.

    Multistate Outbreak of Listeriosis Linked to Imported Frescolina Brand Ricotta Salata Cheese

     

    Recall

    On September 10, 2012, Forever Cheese, Inc. voluntarily recalled one lot of Frescolina brand ricotta salata cheese due to Listeria monocytogenes contamination.

     

    The recalled cheese was sold to distributors for retailers and restaurants in California, Colorado, District of Columbia, Florida, Georgia, Illinois, Indiana, Maine, Maryland, Massachusetts, Montana, New Jersey, New Mexico, New York, Ohio, Oregon, Pennsylvania, Virginia, and Washington between June 20, 2012 and August 9, 2012.

     

    The recall affects lot number T9425 and/or production code 441202. Products were sold to supermarkets, restaurants and wholesale distributors.

     

    Advice to Consumers

    Contaminated ricotta salata cheese has made people sick.

     

    •CDC recommends that consumers not eat imported Frescolina brand ricotta salata cheese. This is especially important for pregnant women, persons with weakened immune systems, and older adults.

    •Even if some of the cheese has been eaten without anyone becoming ill, the rest of the cheese should be disposed of immediately.

    •When in doubt, throw it out. (See disposal recommendations below)

    How do I recognize the contaminated cheese?

     

    •The recalled cheese is labeled Frescolina brand ricotta salata cheese. It is a product of Italy. Consumers should consult with their retailer if they have questions about the origin of cheese.

    •Be aware that the recalled cheese has a 4 month shelf life, so contaminated cheese may still be in consumer’s refrigerators and may still be for sale in stores.

    •Ricotta salata cheese is often served crumbled or grated as an ingredient in some dishes.•Ricotta salata is a salty white cheese made from pasteurized sheep’s milk. Note: It is not the same as ricotta cheese, which is a very soft cheese often sold in plastic tubs.

    Contaminated cheese may still be in consumers' homes.

    Listeria can grow in cut cheese at room and refrigerator temperatures. Listeria can also spread to other cheeses cut and served on the same cutting board or stored in the same area.

     

    •Follow these simple steps if recalled cheese is or was in your home:

    ◦Dispose of cheese in a closed plastic bag placed in a sealed trash can. This will prevent people and animals from eating it.

    ◦Wash the cheese drawer and other areas where the cheese was stored in the refrigerator with hot water and soap. Sanitize the area with a solution of one tablespoon of chlorine bleach to one gallon of hot water.

    ◦Wash cutting boards, surfaces, and utensils used to cut, serve, or store cheese. If possible, use a dishwasher; otherwise, use hot water and soap, followed by sanitizing with a dilute bleach solution described above.

    ◦Wash hands with warm water and soap following the cleaning and sanitation process.

     

    Who is most at risk?

     

    •Listeriosis primarily affects pregnant women and their newborn infants, persons with weakened immune systems, and older adults.

    •Persons who think they might have become ill from eating contaminated cheese should consult their doctor immediately. People can sometimes develop listeriosis even up to two months after eating contaminated food.

    Legrand Wiremold Recalls Power Strips Due to Electric Shock Hazard

Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

     

    Name of Product: Legrand Under Cabinet Power and Lighting four outlet power strip

     

    Units: About 14,200

     

    Importer: Legrand Wiremold, West Hartford, Conn.

     

    Hazard: The electrical wires are reversed on the receptacles on the power strips, posing a risk of electrical shock.

     

    Incidents/Injuries: One incident was reported. No injuries were reported.

     

    Description: This recall involves four-outlet under-cabinet power and lighting strips model number PX1001. The power strips come in a red package with "Wiremold-Series Part PX1001" printed on the front. The manufactured dates of the recalled products are presented as product date codes January 2011 through June 2012, and are found imprinted on a circle on the bottom with an arrow pointing to the number of the month and the year "11" or "12" in the center. The model number PX1001, and UPC number 0 86698 00125 3, are imprinted on the bottom of the packaging. Power strips that have a star symbol printed in black ink on the packaging and on the back cover of the power strip are not included in this recall.

     

    Sold at: Ace Hardware, Do it Best, Home Depot USA, Sutherland, True Value Hardware and online at Amazon.com between February 2011 and August 2012 for about $40.

     

    Manufactured in: China

     

    Remedy: Consumers should immediately unplug and stop using the power strips and return them to Legrand Wiremold for a replacement or refund.

    Death of Child Prompts Recall of Window Blinds by Blind Xpress

    Two-year-old strangles in cord

     

    WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Blind Xpress of Livonia, Mich. is announcing the recall of about 139,000 custom-made vertical and 315,000 horizontal blinds. In 2009, a 2-year-old girl from Commerce Township, Mich. reportedly strangled in the loop of a vertical blind cord that was not attached to the wall or floor.

     

    Blind Xpress custom vertical blinds have an adjustment cord that forms a loop that is not attached to the wall or floor. In some instances, this loop has a weighted device at the bottom. The custom horizontal blinds do not have inner cord stop devices to prevent the accessible inner cords from being pulled out. A child can become entangled in a cord loop and strangle.

     

    This recall involves all Blind Xpress custom-made vertical blinds that do not have a cord-tensioning device that attaches to the wall or floor, as well as all horizontal blinds that do not have inner cord stop devices.

     

    The blinds were sold at various blind specialty stores in Michigan, Ohio and Indiana from January 1995 through December 2011 for between $16 and $380. These blinds were manufactured in the United States.

Shake'Ems seasoning cups distributed in movie theaters in California, Colorado, Arkansas and Tennessee. People who have an allergy or severe sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume these products.

     

    The products were distributed in a .5 oz. plastic cup labeled with Shake'Ems on the top. Flavors include: White Cheddar, Nacho Cheddar, Ranch, Cinnamon Sugar and Parmesan Garlic.

     

    No illnesses have been reported to date in connection with this problem.

     

    The recall was initiated after the FDA discovered that the products contain milk and red 40, but product packaging did not reveal the presence of these ingredients. Subsequent investigation indicates the problem was caused by a temporary breakdown in the company’s labeling process.

     

    Production of the product has been suspended until the FDA and the company are certain that the problem has been corrected.

    FOR IMMEDIATE RELEASE - September 4, 2012 - Sun Pharmaceutical Industries, Inc. (Sun Inc) announced today that it is voluntarily recalling from users one lot of Nimodipine Capsules, 30 mg, marketed by Caraco Pharmaceutical Laboratories, Ltd. Sun Inc. commenced the recall as a precautionary measure due to the presence of crystals of nimodipine within the capsule solution of this lot as identified by a customer complaint. No adverse events have been reported at this time.

     

    Nimodipine Capsules, 30 mg, are used to decrease problems due to subarachnoid hemorrhage (bleeding in the brain). Nimodipine capsules 30 mg are clear yellow solution filled in oblong opaque light yellow softgel capsules, imprinted “135” in black ink. The product is supplied in unit dose blisters of 30 and 100, as described below. The affected product was distributed nationwide between January 19, 2012 and April 24, 2012

     

    The crystallization of the nimodipine fill material in the capsule could adversely affect the product’s bioavailability. Although clinical health implications are unknown, use of the product when the nimodipine has crystallized in the capsule may be of great clinical significance. The product may no longer be bioequivalent and may potentially affect patients who are being treated for a medical emergency.

     

    As a precautionary measure, Sun Inc is recalling the following lot numbers to the consumer level to minimize any potential risk to patients

     

    Lot Number: 3305.039A, NDC Number: 57664-135-65 (Unit Dose Blisters of 100 (25x4))

     

    Lot Number: 3305.039B, NDC Number: 57664-135-64 (Unit Dose Blisters of 30 (5x6))

     

    The recalled capsules were manufactured for Sun Inc by Pharmaceutics International, Inc. This recall is being conducted with the knowledge of the Food and Drug Administration.

     

    Patients and healthcare providers using Nimodipine Capsules, 30 mg, with one of the above lot numbers should discontinue use of the product and should contact the following number for more information about the recall: Inmar Inc. at 1-800-967-5952 (Option 1 then Option 3). Representatives are available Monday through Friday, 8 AM to 5 PM EST.

  10. When we would eat them fresh, my mother would cook them with a couple slices of bacon for flavoring. They did't cook near as long as dried ones.     FOR IMMEDIATE RELEASE - August 30, 2012 - World Foods, LLC is initiating a voluntary, precautionary recall on various products it distributes to retail supermarkets that contain mangoes associated with the Splendid Products recall of Daniella Brand Mangoes with the potential to be contaminated with Salmonella.

     

    Salmonella, is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

     

    There have been no reported illnesses attributed to the recalled items. World Foods, LLC, Florida is issuing this voluntary recall linked to Daniella Brands Mangoes to minimize risk to the public health.

    WASHINGTON, Aug. 30, 2012 – Klement Sausage Company Inc., a Milwaukee, Wis. establishment, is recalling approximately 2,920 pounds of frozen bratwurst patties because they may contain foreign materials—pieces of a plastic pen, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.

     

    The products subject to recall include:

     

    10-lb. cases containing 4-ounce patties of KLEMENT SAUSAGE CO. BRATWURST PATTY.

     

    All products were produced on July 6, 2012. The packages bear the establishment number “EST. 2426B” in the USDA mark of inspection and the batch number “21097” on the case label. The products were distributed for foodservice use in Iowa, Kentucky, Minnesota and Wisconsin.

     

    FSIS was alerted to the problem by the firm after the company received complaints from distributors who were made aware by food preparation personnel who discovered the foreign matter while preparing to cook the product. FSIS and the company have received no reports of injury or illnesses associated with consumption of this product. Anyone concerned about an illness should contact a healthcare provider.

     

    FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers.

    FOR IMMEDIATE RELEASE - August 30, 2012 - Brand New Energy ("BNE"), dietary supplement re-sale distributor, is recalling all lot codes of EphBurn 25. The recall was initiated on August 28, 2012, after notification by the Food and Drug Administration (FDA) to a third-party retailer which purchased EphBurn 25 that one lot of EphBurn 25 was sampled by the FDA and found to contain ephedrine alkaloids, making it an unapproved drug.

     

    Ephedrine is commonly used as a stimulant, appetite suppressant, concentration aid, and decongestant, and it has been used to help aid in weight loss. The ephedrine alkaloids work mainly by increasing the activity of noradrenaline on adrenergic receptors. A number of adverse effects associated with ephedrine alkaloid-containing dietary supplements have been reported to the FDA. These include elevated blood pressure, rapid heartbeat, nerve damage, muscle injury, and psychosis and memory loss. More serious effects have also been reported, including heart attack, stroke, seizure and death. There have been no reports of adverse events associated with this recalled product.

     

    This recall affects all lot codes and use by dates of EphBurn 25. The product is a 90-count bottle with red capsules and prominently displays the product name "ephBURN 25" in white letters on the front of a red label. There is no UPC code. EphBurn 25 was previously discontinued on or about May of 2012.

     

    BNE is a reseller of nationally-known diet and energy supplements such as Zantrex 3, Trim Spa, Hydroxy Cut and others. The product subject to recall, EphBurn 25, was distributed to various retail stores nationwide, and the product was sold via the Internet from the period of time of approximately April 2010 through August 2012. No other products distributed by BNE are subject to recall.

     

    Consumers who may have purchased EphBurn 25 should immediately discontinue using the product and contact their health care professional if they have experienced any adverse effects. Consumers can contact the distributor of the product at info@brandnewenergy.com or call 1-888-234-2595 (8 a.m. to 4 p.m. PST) to receive further instructions for returning the product or with any questions.

     

    We sincerely regret any inconvenience to consumers. This recall has been taken voluntarily out of concern for the health and safety of consumers.

    Meijer Stores Recalls Bicycles Due to Fall Hazard

    Name of Product: Various models of Huffy, Iron Horse, Mongoose, Northwoods, Pacific, Razor and Schwinn bicycles

     

    Units: About 68,000

     

    Retailer: Meijer Inc., of Grand Rapids, Mich.

     

    Hazard: Pedals on the bicycles can loosen or detach during use, posing a fall hazard to the rider.

     

    Incidents/Injuries: There are 29 reports of pedals detaching or coming loose during use, including 16 reports of minor injuries.

     

    Description: This recall involves two-wheeled youth and adult Huffy, Iron Horse, Mongoose, Northwoods, Pacific, Razor, and Schwinn bicycles measuring 20" or more. They were assembled on-site at Meijer stores by Serv-U-Success of Grandville, Mich. The bicycles were sold between March 2012 and August 2012 and have a Serv-U-Success assembly sticker attached. This recall does not include bicycles with Serv-U-Success assembly stickers written in green marker. The Serv-U-Success assembly sticker is located on the bottom of the bicycle frame between the pedals or on the back of the frame facing the rear tire.     Sold exclusively at: Meijer stores in Michigan, Ohio, Illinois, Indiana and Kentucky from March 2012 through August 2012 for between $60 to $300. Bicycles sold at Michigan Meijer stores in Cadillac, Gaylord, Petosky and Traverse City are not included in this recall.

     

    Remedy: Consumers should immediately stop using the recalled bicycles and return them to any Meijer store for a full refund or a replacement bicycle of the same type and value. Consumers will also receive a $10 store coupon.

    Name of Product: Bicycle Brake Levers

     

    Units: About 100 (about 600 of the 2010 and 2011 Shiv Modules and 2012 Shiv TT modules were recalled in April 2012).

     

    Importer: Specialized Bicycle Components Inc., Morgan Hill, Calif.

     

    Hazard: The adjuster cap and brake cable can slide out of position and make the brakes non-operational. This can cause a rider to lose control of the bicycle and crash.

     

    Incidents/Injuries: Specialized is aware of one incident worldwide in which the rider lost the function of both brakes. The firm has received no reports of injury or property damage.

     

    Description: This recall includes Tektro TL-83 brake levers sold on 2012 S-Works Shiv bicycle frame modules, 2012 S-Works Shiv TT bicycle frame modules, and sold as aftermarket service parts for these modules. The TL-83 is a version of the TL-720 brake lever, modified with a quick release slot at the top of the lever arm and designed exclusively for use with aerodynamic handlebars (aerobars) sold as original equipment on these modules. They are black aluminum and model number "TL-720" can be read on the side of the lever arm when braking action is applied.

     

    Sold at: Authorized Specialized Bicycle Components retailers from May 2011 to June 2012. The bicycle frame modules sold for between $5,500 and $6,100. The brake levers sold for about $80 as service parts.

     

    Manufactured in: China

     

    Remedy: Consumers should immediately stop riding modules equipped with TL-83 brake levers and return the levers or modules to an authorized Specialized Bicycle Components retailer for free replacement brake levers.

    Description: The recycled silk floor mats measure 2 ft. by 3 ft. They were sold in a variety of color stripe combinations. "World Market" and "SKU 441536" are printed on the mat's white care label sewn into the bottom of the mat.

    Name of Product: Snowpulse Avalanche Airbags

     

    Units: About 1,200 in the United States and 2,600 in Canada

     

    Importer: Mammut Sports Group Inc., of Shelburne, Vt.

     

    Manufacturer: Snowpulse SA, of Martigny, Switzerland

     

    Distributor: Mountain Sports Distribution, of Golden British Columbia, Canada

     

    Hazard: A leak in the airbag's cartridge can result in the airbag not deploying, posing a risk of death and injury in the event of an avalanche.

     

    Incidents/Injuries: None reported.

     

    Description: This recall involves Snowpulse Avalanche airbags with inflation-system 1.0 air cartridges. The airbags are used for skiing, snowmobiling and mountain climbing to help keep the user above the surface if an avalanche occurs. Model year 2008 to 2010 airbag cartridges are included in this recall. The packs are between 15 and 45 liters and have the "Snowpulse" logo printed on them. The metal cartridge is inside the pack and unscrews from the airbag. Cartridges using inflation system 1.0 gauges can be identified by the pin inside the threaded fitting on the side of the cartridge. If this gauge does not have an "A" or a "B" on the dial then it is included in the recall.

     

    Sold by: Specialty outdoor and motorsports stores nationwide from September 2008 through April 2012 for between $900 and $1,200.

     

    Manufactured in: Switzerland

     

    Remedy: Consumers should immediately stop using the recalled airbags and contact Snowpulse for a replacement cartridge.
